Must have active operating authority registered with the FMCSA.

Minimum $1,000,000 Auto Liability and $100,000 Cargo Insurance naming MAB Dispatchers as certificate holder.

Current signed and dated W-9 matching your corporate or sole proprietorship filing.
Factoring Notice of Assignment (NOA)
Official bank remittance instructions from your factoring company like Thunder Funding, RTS, Apex, Triumph (or voided check if self-invoicing).
